Job description
Northeast Healthcare Recruitment, Inc. (NEHCR) seeks a Family Medicine Nurse Practitioner or Physician Assistant to join our care team. The role delivers evidence-based primary care and partners with patients and the care team to create individualized care plans that align with the medical home model.
You will provide clinical leadership and guide comprehensive care on-site. You will treat 10–15 patients daily, work with the Whole Health Coach, and participate in annual health evaluations,
