Job description
Community Care Physicians seeks a Family Medicine Nurse Practitioner/Physician Assistant to provide comprehensive primary care in our outpatient practices across New York’s Capital Region. In this patient-centered, team-based role, you’ll evaluate and manage acute and chronic conditions, deliver preventive care, educate patients and families, and coordinate services within a large, physician-owned multispecialty group. You’ll work closely with collaborating physicians and support staff in a culture that values quality improvement, work–life balance, and long-term relationships with patients and colleagues. Opportunities exist to grow across multiple locations and specialties.
