Job description
Community Health Connections is seeking a Staff Nurse in Family Medicine (RN or LPN) to join our outpatient primary care team in Massachusetts. The role focuses on comprehensive, patient-centered care across the lifespan, health promotion, and care coordination with providers and the care team.
Responsibilities include intake, assisting during exams, administering medications and immunizations, documentation in the EHR, and participating in care coordination and patient education.
