Position Summary\ N A well-established medical practice is seeking a full-time Primary Care / Family Medicine Physicians to join a collaborative care team in a community-based outpatient setting. This opportunity is ideal for a physician who enjoys providing comprehensive care to patients of all ages and values continuity of care within a supportive practice environment.

Responsibilities\ N Evaluate, diagnose, and treat patients using appropriate medical judgment and evidence-based practices
\ N Provide comprehensive primary care services across the lifespan
\ N Coordinate patient care, including referrals, follow-up, and care transitions
\ N Participate in quality improvement initiatives to enhance patient outcomes and cost-effective care
\ N Maintain accurate and timely clinical documentation that meets regulatory and payer requirements
\ N Participate in inpatient and outpatient consultation and call coverage as needed
\ N Engage in academic, educational, or research activities as requested
\ N Requirements Certifications & Licensure\ N Board Certified or Board Eligible in Family Medicine or Primary Care specialty
\ N Active state medical license or eligibility to obtain licensure
\ N
Required Education\ N Successful completion of an ACGME-accredited residency program
