Job description
HealthLinc in Mishawaka, IN is seeking a physician to coordinate proactive care for patients with chronic conditions and to identify wellness and preventative services.
This role works with clinical staff, reports to the Site Medical Director, and emphasizes collaboration, patient education, and comprehensive documentation.
Qualifications include medical school graduation, residency, board certification, Indiana license, CPR, and ongoing CME; 1–3 years of experience preferred.
