Job description
Lorain County Health & Dentistry (LCH&D) is seeking a Family Medicine Physician to provide primary clinical care within the PCMH/NCQA model for patients in Lorain County. The successful candidate will build long-term relationships, offer patient-centered plans, and collaborate with a multidisciplinary team to promote wellness and continuity of care.
Requirements include MD or DO, board certification in Family Medicine, Ohio license, BLS, and ongoing CME.
