Family Medicine Physician
Saratoga Springs, New York
365 Healthcare is recruiting a Board Certified Family Medicine Physician for a full-time permanent outpatient opportunity in Saratoga Springs, New York.

Position Snapshot
Base Salary: $250,000 – $260,000 + productivity incentives
Sign-On Bonus: Up to $75,000
Retention Bonus: $25,000 (Year 2)
Schedule: 4- or 5-day work week
Hours: 40 hours/week (34 patient-facing)
Patient Volume: 18–24 patients per day
Employment Type: Full-time, permanent
Schedule & Work-Life Balance
Flexible 4- or 5-day schedule
No in-office weekend hours
Call: Approximately 1 in every 70 days Phone call only
RN triage handles first contact
Telemedicine coverage: 2–3 hours on a weekend once every 3 months
This is a true lifestyle-friendly outpatient role with very low call burden.

Clinical Responsibilities
Provide comprehensive primary care to patients of all ages
Manage chronic and acute conditions
Perform routine exams and preventive care
Collaborate with APPs and care teams
Maintain accurate documentation in Epic
Deliver patient-centered, evidence-based care
Qualifications
Board Certified or Board Eligible in Family Medicine
Active New York license (or ability to obtain)
DEA and ACLS certification (by start date)
Strong interest in full-spectrum primary care
Clean malpractice and license history
Compensation & Benefits
$250K–$260K base + productivity incentives
