Job description
Sherburne Family Practice is seeking a compassionate, mission-driven Primary Care Physician to join our dedicated team providing high-quality, patient-centered care to individuals and families in the Sherburne community. This is a full-time opportunity within a supportive practice environment focused on collaboration, continuity of care, and long-term patient relationships.
Primary Department, Division, or Unit:
UHS Primary Care Norwich
Annual Compensation Range:
$275,000-$345,000
Responsibilities
Provide comprehensive primary care to patients across the lifespan
Conduct routine wellness exams, chronic disease management, and acute care visits
Collaborate with a multidisciplinary team to ensure high-quality patient outcomes
Maintain accurate and timely medical records
Participate in quality improvement initiatives and clinical meetings
Promote preventive care and patient education
Qualifications
MD or DO degree from an accredited medical school
Board Certified or Board Eligible in Family Medicine or Internal Medicine
Active or eligible for New York State medical licensure
Strong communication skills and a commitment to patient-centered care

Salary: $275,000 - $345,000 annually
