Job description
Community Health Connections seeks a Primary Care Physician (Family Medicine or Internal Medicine) to provide comprehensive, continuous care to a diverse patient panel in Fitchburg, MA.
You will practice in a collaborative model with nurse practitioners and clinical psychologists, prescribing medications, performing exams, and coordinating care across services to improve patient health outcomes. Strong communication and on-call participation are required.
