nFamily Nurse Practitioner (APRN) (Full-time; 40 hours/week)
nLocation: Willimantic, CT (Medical Department)
nGenerations Family Health Center, Inc. is a regional community health center covering a thirty seven town rural region in eastern Connecticut with sites located in Willimantic, Danielson, Norwich and Putnam. Our dedicated staff provides a full range of primary care services to all ages including medical, dental and behavioral health care, as well as care coordination and community outreach. In addition to our main facilities, we offer mobile programs and School Based Health Centers.
nOur Community - Generations Family Health Center:
nFamily Nurse Practitioner Role: Provide a broad range of clinical primary care services, which will include routine and appropriate ambulatory adult, pediatric, and gynecological care as assigned.
nKey responsibilities include:
nnnProviding care appropriate to the age of the patients served
n
nnPerforming office diagnostic and therapeutic procedures
n
nnPerforming therapeutic procedures such as injections, immunizations, suturing, wound care, and managing infection
n
nnInstructing and counseling patients regarding compliance with prescribed therapeutic regimens, normal growth and development, family planning, emotional problems of daily living, and health maintenance
n
nnDeveloping and implementing patient management plans, recording progress notes, and assisting in provision of continuity of care
n
nnPerforming comprehensive physical examinations and compiling patient medical data, including health history and physical exam results
n
n nQualifications
nQualifications:
nnnM.S. or D.N.P degree from an accredited school of nursing
n
nnBoard certification
n
nnCurrent licensure in the State of Connecticut or ability to obtain license in the state
n
nnValid Federal and State Controlled Substance License
n
n nCompensation: $116,000 - $131,000 (based on applicable experience)
nBilingual Stipend for Spanish or Haitian Creole fluency (upon passing proficiency assessment)

nnnPrimarily day-time schedule, some evenings (8 hour shifts, Monday-Friday) / one Saturday per month (4 hour shift, stipend)
