Job description
Community Health Connections (CHC) in Gardner, MA is seeking a Nurse Practitioner to coordinate comprehensive patient care under physician supervision. You will perform assessments, diagnose conditions, prescribe medications, and participate in quality improvement initiatives across CHC sites.
The ideal candidate holds MA RN licensure and FNP certification, with CPR/AED and 3–5 years of nursing experience in a clinical setting. CHC offers a supportive, mission-driven environment.
