Your Responsibilities
Conduct thorough patient evaluations, order diagnostic assessments, and develop tailored care plans
Diagnose and manage acute, chronic, or episodic illnesses independently or in collaboration with physicians (based on credentialing)
Guide patients and families in lifestyle modifications, wellness strategies, and disease management
Prescribe medications, therapies, and interventions as per the scope of practice
Participate in patient follow-ups and chronic disease management initiatives
Collaborate with multidisciplinary teams to ensure comprehensive, patient-centered care
Maintain detailed and accurate documentation within electronic medical records (EMR)
Qualifications
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
Completion of an accredited Nurse Practitioner program
Licensure & Certifications
Valid Nurse Practitioner license in Arizona
National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or equivalent
Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the ability to obtain within 90 days)
