Healthcare the Way It Was Meant to Be

Are you a Nurse Practitioner who wants to spend more time caring for patients and less time fighting the healthcare system? Hope + Health Direct Primary Care is a growing, Nurse Practitioner-owned and operated practice looking for the right Family Nurse Practitioner to join our team.

Our practice is built around a simple idea: when you take away the insurance-driven bureaucracy, rushed visits, and productivity quotas, you can get back to what matters most—the patient and your quality of life. You will have the time to spend with your patients to ensure they get the best care, without having to sacrifice your time away from work on paperwork.

Our founder has been practicing in this community as a Nurse Practitioner since 2009 and has worked in Direct Primary Care since 2018. We understand the value of autonomy, flexibility, meaningful patient relationships, and creating a practice environment where providers can actually enjoy caring for their patients.

We are not looking for someone to simply fill a schedule. We're looking for the right person to become part of our team. We value trust, communication, flexibility, and work-life balance.

Who We're Looking For

The ideal candidate is a clinician who is:

Honest and deeply committed to integrity — You practice ethically, communicate openly, and put the patient's safety and best interest first.

Comfortable with autonomy — You are confident in making clinical decisions and managing your patients independently within the scope of Texas NP regulations.

A true self-starter — You see what needs to be done, take initiative, and don't need someone constantly looking over your shoulder.

Patient-centered — You believe healthcare should be personal, thorough, preventative, and relationship-based.

Collaborative — You enjoy being part of a close-knit team while still appreciating the freedom to practice independently.

Adaptable and growth-minded — You enjoy being part of a growing practice and is willing to help improve systems, workflows, and the patient experience.

What You'll Do

As a Family Nurse Practitioner at Hope + Health, you will provide comprehensive primary care to patients across the lifespan, including:

Performing comprehensive physical examinations and health assessments

Diagnosing and treating acute and chronic medical conditions

Managing ongoing preventative and primary care needs

Ordering, reviewing, and interpreting laboratory and diagnostic testing

Developing individualized, evidence-based treatment plans

Prescribing medications within your scope of practice and Texas regulations

Providing patient education and counseling on preventative health and lifestyle

Coordinating referrals and collaborating with specialists when needed

Using telemedicine and secure messaging to improve patient access and continuity of care

Maintaining accurate, timely, and thorough medical records

Building long-term relationships with patients and their families

Contributing ideas and helping us continually improve the way we deliver care

What We're Looking For

Required:

Active, unrestricted Nurse Practitioner license in Texas

Board certification through AANP, ANCC, or applicable national certifying organization

Strong clinical judgment and communication skills

Comfort working independently seeing patients in-office, virtually and using various technology

Minimum of 3 years+ of experience in primary care, urgent care, or emergency medicine. If less than this, you will need a very strong nursing background as you will work very autonomously at times.

Experience caring for patients across the lifespan
