Job description
New Living Healthcare Services is seeking a dedicated Family Nurse Practitioner (FNP) to join our multidisciplinary team delivering patient-centered care across primary care, geriatrics, and behavioral health.
You will assess, diagnose, and manage health conditions, develop individualized care plans, collaborate with behavioral health providers, and participate in quality improvement, education, and care coordination.
