Job description
Health Solutions is seeking a Family Nurse Practitioner to join their Medical Services Team in Pueblo, CO. You will provide comprehensive primary care for adults and children, coordinate care, and participate in quality initiatives within a PCMH framework.
Under supervision, you will work with physicians, nurses, and care coordinators to deliver patient-centered care, manage chronic conditions, and improve health outcomes while adhering to state regulations and organizational policies.
