Job description
We are seeking a dedicated and compassionate Board-Certified or Board-Eligible Family or Internal Medicine Physician to join our team in Colebrook, NH. This full-time, outpatient position involves providing comprehensive, high-quality primary care to a diverse patient population across all ages, with a particular emphasis on chronic disease management and care for an aging population. Physicians maintain their own patient panels and are supported by a collaborative clinical team, including an RN team leader and a medical assistant.
What We Offer:
Salary $250,000 - $300,000 with potential wRVU productivity and quality bonuses

Schedule & Practice Details:
Outpatient only, full-time (36 patient contact hours + 4 admin hours)
Typical patient volume: 16–22 patients per day
EMR: Meditech Expanse with Dragon dictation
May participate in day‑time call rotation (no after‑hours call)
Clinical support from RN team leader and MA
Requirements:
MD or DO with Board Certification or Board Eligibility
Successful completion of an accredited residency (ACGME, AOA, or RCPSC)
Current ACLS and BLS certification through the American Heart Association
Comfortable managing a personal patient panel and working independently
Strong experience in chronic disease management, especially with an elderly population
