Practice Highlights:
Predominately outpatient clinical role with meaningful medical director leadership opportunities
No call, no inpatient responsibilities and no home care
Small, intentionally designed patient panel that supports high-quality geriatric care
Opportunity to help shape the geriatric care model across primary care and consult services
Operational dyad partnership to advance clinical priorities, care pathways and team development
80% clinical (PCP + geri consult clinic) 20% leadership, supported by an operational dyad
Role & Scope of Practice:
Deliver outpatient geriatric primary care and consult services
Partner in a dyad structure to translate clinical priorities into operations
Expand the team by adding 2 APPs with leadership support
Collaborate with palliative care and specialty partners to strengthen care pathways
