Job description
Primary Care Physician
Locations: Dartmouth, Leominster or Springfield, MA
Compensation: $275,000 $300,000 annually, based on experience

Low Patient Volume: Average of 4 patients per day with a panel of 65

Mission-Focused Practice: Help frail elders remain safely in their homes and avoid nursing home placement
Professional Support: Strong peer mentorship and leadership engagement
Key Responsibilities
Provide primary, preventive, urgent, and chronic care to PACE participants
Conduct initial and biannual assessments and contribute to individualized care plans
Participate actively in interdisciplinary team (IDT) meetings
Coordinate specialty care and transitional care services
Deliver care across settings including clinic, home, and inpatient facilities
Participate in limited after-hours call rotation
Maintain timely and accurate clinical documentation
Qualifications
MD or DO licensed in Massachusetts; DEA required
Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatrics (geriatrics preferred)
