Family Practice PA or FNP Canby Clinic
Full Time 4 Day Work Week Option 12-14 Patients/Day
The Canby Clinic is seeking a dedicated Physician Assistant (PA) or Family Nurse Practitioner (FNP) to join our family practice team. We are looking for a provider who is committed to long term community care and ideally open to a 5 year commitment. This role offers stability, autonomy, and a supportive, multidisciplinary environment.
Key Details:
Full scope family practice (all ages)
12 patients per day expected
Scheduled for 14 to account for no shows
EMR: Cerner Community Works
Strong on site support from physician and other healthcare professionals
Credentialing normally takes 1 month.
- a lot of women's health and would love for a female provider to do that work if possible.
- have an after hours call number for the clinic but that call is split between all providers so maybe one week out of very 8 or 9 weeks they would carry the after hours call phone. Call volume is very low maybe 2-3 calls per week.
Schedule & Workload
4 day work week available
Providers are scheduled for 8 hours of patient care per day
Paid for 10 hours/day, including 2 hours of protected admin time

Test result follow up
Paperwork and care coordination
Compensation
Annual salary: $144,830.40 $214,510.40 (based on full time schedule)
Employment agreements structured with a daily rate, applied to worked days and PTO days
