Job description
COMPENSATION:
Salary $150k+

Role:
Compensation:

13-16 patients a day

Administrative time included in schedule
No nights or weekends
100% outpatient
Dedicated Care and support team
Patient Population: Seniors with 3-5 chronic diseases
