Job description
Salary Range: $130,000-$175,000
A well-established multispecialty practice is seeking an experienced Nurse Practitioner to deliver high-quality outpatient primary care. This role offers the chance to build lasting patient relationships in a collaborative, physician-led environment. You'll be supported by an integrated network of primary care physicians, cardiologists, endocrinologists, and other specialists.
You'll step into an established patient population rather than building a panel from scratch. The practice also runs a dedicated same-day service for acute needs, and its internal specialty network keeps referrals efficient and communication close. That means you can focus on thorough, continuous care.
Responsibilities
Conduct comprehensive patient evaluations and physical examinations
Manage acute visits for a broad range of primary care concerns
Provide ongoing chronic disease management, including conditions such as hypertension, diabetes, and hyperlipidemia
Deliver preventive care, including screenings, wellness visits, and patient education
Order and interpret diagnostic tests, labs, and imaging
Prescribe and manage medications
Coordinate specialist referrals and collaborate closely with physicians across the network
Document care accurately and in a timely manner in the EHR
Qualifications:
Board certification: FNP or AGNP in New York State
Active DEA registration, or eligibility to obtain one
Outpatient primary care experience preferred
Current BLS certification
Strong clinical judgment, communication skills, and a patient-centered approach
