A healthcare organization is seeking an experienced Family Nurse Practitioner to join an employer-sponsored primary care clinic in Hammond, Indiana. This opportunity offers a four-day workweek, low patient volume, and the ability to provide comprehensive primary care in a setting focused on quality patient outcomes rather than productivity metrics.
This role is ideal for providers who enjoy preventive medicine, wellness visits, and developing long-term relationships with patients and families.

Practice Setting Providers work in a modern outpatient clinic serving employees and their families. Unlike traditional fee-for-service practices, the focus is on preventive care, wellness, chronic disease management, and continuity of care, allowing providers more time with each patient.
Clinical Responsibilities Conduct annual physicals and routine wellness exams
Diagnose and treat acute and chronic conditions
Provide primary care services for pediatric and adult patients
Perform women's health services, including Pap smears and preventive screenings
Administer immunizations and vaccinations
Order and review laboratory studies
Coordinate care with specialists and community providers
Educate patients on healthy lifestyle choices and disease prevention
Document encounters utilizing Epic EMR
Schedule Tuesday & Wednesday: 9:00 AM - 7:00 PM
Thursday & Friday: 6:00 AM - 4:00 PM
38 hours per week
Weekday schedule only
No weekends
Start Date: October 2026
12-week contract with extension potential
Patient Volume & Support Average 7-12 patients per day
RN/MA support available

Qualifications Required Board-Certified Family Nurse Practitioner (FNP)
Active Indiana Nurse Practitioner License
Active DEA Registration
Current BLS Certification
Minimum 2 years of outpatient primary care experience
Experience treating patients of all ages
Preferred Epic EMR experience
Employee Health experience
Occupational Health experience
Strong preventive care and wellness focus
Experience with pediatric and adult populations
