Millennium Physician Group, part of Mosaic Health, is seeking an experienced and compassionate Nurse Practitioner (APRN) or Physician Assistant (PA-C) to join our growing primary care team in Jacksonville, Florida .

What We re Looking For
Current Florida APRN or Physician Assistant license
BLS/ACLS certifications
Primary care experience preferred
Commitment to patient-centered, value-based care
Quality Measures/ HCC's
