Position Details
Setting: 100% Outpatient - Adult Medicine (18+)
Schedule: Full-time, weekdays only. Flexible Schedule
Call: No call required
What You'll Do
Provide comprehensive primary care services to patients ages 18+
Conduct routine physical exams and health assessments
Diagnose and manage acute and chronic conditions
Develop individualized treatment, education, and care plans
Collaborate closely with physicians and interdisciplinary care teams
Coordinate referrals to specialists as needed
Deliver high-quality, patient-centered care in a supportive environment

Qualifications
Education: APRN - Adult or Family Nurse Practitioner
Licensure: Eligible for or active Connecticut APRN license
Experience: Minimum 1 year in a primary care or outpatient setting preferred
