Job description
Join a patient-centered, value-based care organization where the focus is on quality outcomes, preventive care, and meaningful patient relationships rather than visit volume. This full-time opportunity offers a Monday through Friday schedule (8:00 AM to 5:00 PM) serving patients throughout the Antelope Valley, San Fernando Valley, Conejo Valley, and Simi Valley. You'll provide acute, post-acute, and transitional care services in patients' homes, helping individuals successfully recover after hospitalization, manage chronic conditions, and avoid unnecessary readmissions. Supported by a collaborative interdisciplinary team, you'll have the autonomy, resources, and time needed to deliver personalized, relationship-centered care where it matters most. Mileage Reimbursement
Quarterly Incentive Plan (10% of quarterly earnings)

What you need: Active CA Nurse Practitioner license, DEA license, and BLS certification.
