Avg volume: 12 ppd
Patient mix: Ages 14-65
Required procedures: Primary care services, biometric screenings, chronic disease management, women s health (i.e., PAPs)
Support staff: 1 Medical Assistant and 1 Patient Service Representative/Community Engagement Coordinator
EMR system: Athena
Requirements:
Board-Certified Family Nurse Practitioner or Women s Health NP
Active MO license
Must be Medicaid and Medicare enrolled
