Duties & Responsibilities
Provide comprehensive primary care to elderly patients in clinic and community settings.
Perform home visits to assisted living, SNF, and personal residences.
Collaborate with interdisciplinary team (11 members) to coordinate patient care.
Manage patient care via Epic EMR.
Participate in PACE program, focusing on holistic care for over-55 population.
