Nurse Practitioner Primary Care San Jose, CA
Location: San Jose, California
Compensation: $74.88 $86.81/hour
Specialty: Primary Care / Family Medicine
Setting: Outpatient Community Health / Primary Care
We are seeking an experienced Nurse Practitioner to join a multidisciplinary primary care team in San Jose, CA. This position focuses on providing comprehensive outpatient care and may also include women s health, family planning, and other community-based services.

Full-time outpatient position
Community health / primary care setting
Collaborative, multidisciplinary care team
Provide comprehensive primary and preventive care
Opportunity to work with a diverse patient population
Provide patient education and health counseling
Responsibilities Evaluate, diagnose, and treat primary care patients
Perform comprehensive physical assessments and examinations
Develop appropriate treatment and care plans
Prescribe medications within scope of practice
Order and interpret laboratory testing
Provide preventive care and patient education
Coordinate referrals to physicians, specialists, and outside services as needed
Maintain accurate and timely electronic medical records
Collaborate with physicians, nurses, medical assistants, and other members of the care team
Qualifications Current California Nurse Practitioner license
National NP certification through AANP or another recognized certifying body
Completion of an accredited Nurse Practitioner program
Minimum of 2 years of clinical experience
Strong assessment, diagnostic, and patient-care skills
Ability to work independently and collaboratively within a multidisciplinary team
