Job description
A leading Federally Qualified Health Center just southwest of Rutland, Vermont is seeking an experienced Nurse Practitioner or Physician Assistant to provide high-quality primary care across multiple community-based clinic locations while enjoying a collaborative team environment and an exceptional work-life balance.
Position Highlights:
Provide comprehensive primary care across multiple clinic locations
Manage acute, chronic, and preventive care for patients of all ages
Perform comprehensive exams, diagnoses, and treatment planning
Order and interpret labs and diagnostic testing
Perform office procedures, including suturing and wound care
Prescribe medications and provide patient education
Coordinate care with physicians and multidisciplinary teams
Utilize integrated behavioral health and care management resources
Practice within a Level III Patient-Centered Medical Home model
Fully integrated EMR with strong clinical support
0.9 FTE with a 4-day work week
Limited call averaging just 3 weeks per year
Qualifications:
Nurse Practitioner or Physician Assistant
Graduate of an accredited NP or PA program
Active Vermont licensure or ability to obtain
3 5 years of primary care experience preferred
Strong clinical assessment and diagnostic skills
Excellent communication and teamwork abilities
Commitment to high-quality, patient-centered care
Compensation & Benefits:
Competitive salary: $125K $165K
