General Summary Provides diagnosis and treatment of patients within the scope of practice as defined by all applicable Federal, State and local laws, regulations, protocols, etc. and within the individual Advanced Practice Provider's (APP) particular clinical specialty area, licensure and competency level. Performs acts of medical diagnosis or prescription of medical therapeutic or corrective measures as appropriate.

What You?ll Champion:

Seeking a Family Nurse Practitioner (FNP) or Physician Assistant (PA) to support a Community Health Center dedicated to improving access to care in York County.

Provide continuity of care for an existing and growing patient population.

Manage and support an established patient panel, including individuals receiving Medication?Assisted Treatment (MAT).

Help establish care for a vulnerable and underserved patient population in a mission?driven outpatient setting.

Deliver both in?person and virtual primary care services, supporting flexible access for patients.

Collaborate within a supportive care team consisting of four APPs and two Pediatricians.

Qualifications

Minimum Education: Masters Degree Nurse Practitioner Required or

Bachelors Degree Physician Assistant Required

Masters Degree Physician Assistant Preferred

Work Experience: 1 year Required

Licenses: Certified Registered Nurse Practitioner Upon Hire Required or

Licensed Physician Assistant Upon Hire Required and

Basic Life Support Upon Hire Required or

Advanced Cardiac Life Support Upon Hire Required

Courses and Training: BLS and/or ACLS as appropriate for the clinical role. Upon Hire Required and

Nurse Practitioner - board certified by American Nurses Credentialing Center Commission on Certification Upon Hire Required and

Nurse Practitioner - Completion of an accredited program for nurse practitioners Upon Hire Required or

Physician Assistant - Graduate of accredited physician assistant program Upon Hire Required
