APP Post Acute Care opening in Bronx, NY New York CityFull-time, EmployedWe are seeking a quality-driven Advanced Practice Clinician to join our post-acute care team in the Riverdale neighborhood of The Bronx, New York. This is a full-time role (5 days/week) rounding in skilled nursing facilities, with excellent support and training, autonomy, and earning potential.Dedicated night call coverage for work-life balance

Responsibilities: Perform comprehensive patient assessments, including medical histories and physical exams
Develop, implement, and adjust individualized care plans based on patient needs
Monitor progress and coordinate care to optimize outcomes
Collaborate closely with physicians, nursing staff, and other healthcare professionals in a team approach
Educate patients and families on health management and preventive strategies
Maintain accurate, timely medical records in compliance with regulations
Compensation & Benefits Compensation is fee for service with an estimated compensation range of $132,000 - $156,000 annually and no cap on productivity income potential.

Qualifications Experience in post-acute, acute, ED, or clinic settings
Current NP/PA license (State of New York) and DEA
Ability to commute/relocate: Bronx, NY (required)
