The Opportunity In this role, you will evaluate, diagnose, and manage patients in a long-term care setting, providing proactive, comprehensive primary care to a vulnerable geriatric population.

What You ll Be Doing Conduct comprehensive health assessments
Diagnose and manage acute and chronic conditions
Perform medication management and care plan updates
Coordinate care with nursing staff and facility teams
Provide compassionate, culturally sensitive care
Support families through ongoing communication
What We re Looking For Active NP (FNP or AGNP) or PA license
Experience in geriatrics, long-term care, or primary care preferred
