Looking for a primary care role with real variety and deeper patient connections? Join a growing, patient-centered primary care practice serving the Brooklyn community as a Nurse Practitioner, or Physician Assistant in a unique hybrid role blending in-home visits with traditional clinic care.
What You'll Do:
Provide comprehensive primary care in an office/clinic setting, including wellness exams, preventive care, and chronic disease management
Conduct in-home patient visits (typically 2 3 days per week), delivering personalized, home-based care
Evaluate, diagnose, and treat a broad range of acute and chronic conditions across a diverse patient population
Coordinate care with on-site specialists (urology, cardiology, gastroenterology) and ancillary services (PT, mental health, pain management)
Maintain accurate clinical documentation scribe support available for providers who want extra help with EMR
Build deep, long-term patient relationships through ongoing follow-up and support
What You'll Bring:
NP (AGNP or FNP), or PA with active or eligible NY licensure
Excellent interpersonal skills and a strong commitment to patient-centered care
Comfort with both in-office and in-home visit formats
Russian language skills required for this location
