Job description
A leading community health organization in the Green Mountain region of western Vermont is seeking a BC/BE Family Medicine Physician to provide comprehensive primary care while joining a collaborative team dedicated to improving access and delivering exceptional patient care.
Position Highlights:
Provide comprehensive primary care services across all ages
Join an established FQHC serving a large regional patient population
Practice in a Level III Patient-Centered Medical Home
Work alongside integrated behavioral health and care management teams
Leverage strong clinical support and coordinated care resources
Use a fully integrated EMR with robust hospital connectivity
Focus on prevention, chronic disease management, and patient wellness
Participate in quality improvement and population health initiatives
Enjoy a 4-day work week with minimal call responsibility
