Job description
A community-focused healthcare organization in Woodsville, New Hampshire, is seeking a Licensed Clinical Social Worker (LICSW) to provide compassionate, patient-centered psychosocial care as part of an interdisciplinary care team.
Position Highlights:
Conduct psychosocial assessments to identify patient needs and support care planning
Develop individualized treatment plans to support emotional and mental health well-being
Provide individual and group counseling services
Deliver crisis intervention and short-term behavioral health support
Collaborate closely with an interdisciplinary care team
Coordinate care and referrals to community-based medical, psychiatric, and social services
Support patients and families in navigating available resources and care options
Maintain strong relationships with community providers and agencies
Ensure continuity of care through effective care coordination
Contribute to a patient-centered, team-based care environment
