Job description
Institute for Comm Living seeks a dedicated case manager to join its ACT Team in Brooklyn, NY. You will work under supervision to support individuals with serious mental illness in developing stable living, work, and social routines in the community.
You will provide counseling, housing and service referrals, and family education while coordinating with a multi-disciplinary team. This role requires flexible fieldwork and adherence to regulatory guidelines.
