Job description
Lutheran Social Services of Southern California seeks a Facilitator to serve as the hub for coordinating Child and Family Care Plans. You will lead initial meetings, assemble the family team, and guide goal setting, safety planning, and service linkage in a collaborative environment.
You will work with a multidisciplinary team, maintain accurate EHR records, and support families with flexible scheduling including evenings and weekends as needed.
