Job description
Optum, a part of UnitedHealth Group, is seeking a Behavioral Health Advocate to support Medicaid enrollees in Watertown, NY. This field-based role emphasizes care coordination, aftercare planning, and community service linkage to promote stability and reduce hospitalizations.
You will spend ~80% of time meeting members in hospitals, clinics, and homes, with 20% remote work. A NY license (LMHC/LMFT/LCSW/LP) and a valid driver's license are required.
