Job description
Somatus is seeking Field-Based Nurse Case Managers to support high-risk members in homes, dialysis clinics, and physician offices across Queens, NY. This field role requires 75% in the field and close coordination with nephrology practices to improve health outcomes.
Ideal candidates hold an NY RN license with 3+ years of experience, CKD/dialysis exposure, and strong data-entry with MS Office. A BSN or higher is preferred, with telework not implied.
