Job description
Humana Inc. is seeking a part-time Functional Screener to join our Weston/Merrill, WI team.
You will schedule member appointments for functional eligibility re-determinations and conduct in-home, face-to-face interviews using the DHS screening tool to assess needs and determine level of care. Responsibilities include data entry into our online system, timely submission of documentation, and collaboration with staff to ensure accurate records.
