Job description
SCAN Health Plan is seeking an Advanced Practice Clinician to provide primary care to members in their homes, often within senior living communities or skilled nursing facilities. You will perform exams, vaccines, and triage, while building care plans with families and staff.
Responsibilities include coordinating with case management and other healthcare professionals to ensure timely services and optimal outcomes for seniors, with a strong emphasis on quality and compassionate care.
