Job description
Athena Home Health & Hospice, a leading provider of continuum of care services in Southern New England, is seeking a compassionate and skilled Hospice RN Case Manager. You will plan, organize, and direct hospice care services to meet the needs of individuals and families within their homes and communities.
This on-the-road role covers Litchfield County and western Hartford County. Salaried position with weekly pay up to $49 an hour, depending on experience. Shift is Monday-Friday 8a-4:30p.
