Job description
Transitions LifeCare, a North Carolina nonprofit hospice, seeks an experienced Community-Based Hospice RN Case Manager in Durham County. You will lead the care team, advocate for patients, and support families through one of life’s most meaningful journeys.
In this autonomous field-based role, you will develop individualized care plans, deliver nursing assessments and symptom management, and ensure care aligns with physician orders and regulatory requirements while teaching and guiding the care
