Job description
MedZed in Stockton, CA is seeking a Housing Navigator II to help members find and keep housing. The role requires active field work, home visits, and strong collaboration with health plans and landlords to address housing-related needs.
You will assess needs, create service plans, provide crisis support, document in Salesforce Health Cloud, and travel locally to meet members where they live. This non-clinical, field-based position emphasizes a trauma-informed, Housing First approach.
