Job description
Horizontal 2 in San Diego, CA seeks a Housing Navigator II to help members find and sustain housing by assessing needs, planning services, connecting to resources, and following Housing First principles.
This non-clinical, in-field role requires travel to member homes at least four days a week, with documentation in Salesforce Health Cloud, collaboration in interdisciplinary teams, and a commitment to outreach and timely case notes.
