Job description
Humana Inc. (Inclusa team) seeks a part-time Long Term Care Functional Screen Specialist to support Wisconsin Family Care members in the Weston/Merrill area. You will use the DHS screening tool to determine eligibility and needs, conducting in-home interviews and data entry.
The role requires a health/human services degree or an active Wisconsin RN license with relevant experience, plus LTCFS certification. Travel up to 50% and a 28-hour weekly schedule.
