Job description
SCAN Health Plan is seeking an Advanced Practice Clinician to provide and coordinate primary care for seniors in their residences, including in senior living communities or skilled nursing facilities.
You will perform exams, vaccines, chronic disease management, and care coordination with a multi-disciplinary team, while maintaining HIPAA compliance and pursuing continuous quality improvement.
