Job description
Bloom Healthcare in Texas is seeking a field-based Population Health Advanced Practice Provider to join our team. You will perform visits across the Texas territory with a focus on closing gaps and identifying complex conditions for home-based care patients.
You will leverage data and chart discovery to drive improvements in HEDIS and STAR outcomes, collaborating with internal clinicians and external partners to elevate population health.
