Job description
FreedomCare is seeking a Care Manager, RN to perform in-home assessments and coordinate care for members across a MA field territory. This field-based role requires regular travel to patient homes, conducting initial assessments, teaching caregivers, and guiding care plans within the MassHealth AFC framework.
The RN will collaborate with patients, families, and interdisciplinary teams to ensure safe, person-centered care and timely service delivery. MA licensure and driving ability are essential.
