Job description
FreedomCare is seeking a Care Manager, RN for the Connecticut team. This field-based role conducts in-home assessments, orientations for new patients and caregivers, and ongoing care management to ensure safe, person-centered care.
You will travel regularly to patient homes in New Haven and surrounding areas, coordinate with interdisciplinary teams, develop care plans, and ensure regulatory compliance while coaching families and documenting care.
